This patient underwent saline breast augmentation in Orlando, and was obviously unhappy with her initial result. We saw her about 1 year out from her original surgery, but her implants did not appear to be in the right position. Her initial surgeon did not feel that her result was substandard, and felt that the implant position was acceptable.
Upon our evaluation, we felt that the implants were in appropriate position, same as the other surgeon. However, the implant pocket was too restricted, and the pectoralis major muscle was incompletely released. Also, the implants were shifiing very far into her armpit when she flexed her chest muscles. Lastly, her initial implant was a moderate profile implant that was too small a size for her chest diameter.
We undertook a complete capsulectomy on each side, removing the original 225cc moderate plus profile implants, and replaced them with 285cc high profile silicone cohesive gel, “gummy bear” implants. The procedure was done via an inframammary approach. Recovery was six days. Results are shown at 1 year following revision. Note that even though the implant position is lower, there is still superior upper pole fullness due to the higher profile implant.
